Background. Patellar instability is one of the most common pathologies musculoskeletal system, predominantly observed in physically active young individuals. It ranks third structure knee joint injuries after anterior cruciate ligament and meniscal injuries.
 The aim this review to present modern perspectives on diagnosis, principles, surgical treatment techniques for patients with patellar based an analysis literature.
 Methods. Publications were searched PubMed/MedLine eLIBRARY databases. A total 112 foreign articles published between 1984 2023 12 domestic publications from 2011 2022 found. During analysis, 68 selected, which had full texts or abstracts containing sufficient information diagnostic methods, commonly used standard modified correction methods instability, protocols considering patient age structure, characteristics, functional demands.
 Results. qualitatively new stage reconstructive restorative surgery in-depth examination determine extent damage medial retinaculum presence dysplastic changes anatomical structures that provide normal biomechanics extensor apparatus. strictly individual approach selection risk factors contributing development chronic becomes particular importance.
